If you are a partner - an owner of the company - you by default pay whatever the actual real cost of the insurance is. Whether it shows that way on your pay stubs is another matter.
Zero. Zero premium, zero deductible, 100% coverage on pretty much everything. Although we do have a small pharmacy copay, which I find outrageous.
Costs the firm 4k per month
I pay nothing for a family of 4 with incredible benefits, but our firm pays a ton (which I suppose reduces my profit sharing).

$30k+ ; do HSA annual net usually 2/3 lower
In a high deduc plan so hard to say. Max out of pocket for the year would be about 28k. Base monthly is pretty low but can’t remember exactly.
With the “high deductible,” but lower deductible option + vision + dental, about $2K per month for family. I max out the HSA ($9300 in ‘24) in addition to that and used $6500 of HSA in ‘24. (A decent chunk of the out-of-pocket is dental.)

Your getting apples and oranges in here, I assume your asking as part of a full equity position so you would technically be self employed and pay the full cost.
I buy off the marketplace, way better pricing and options. It’s still about $15k and remember this is tax deductible
