I'm 25 and looking for an opportunity as a Data Analyst. I failed 12th once and had 5 backlogs during B.Tech CSE but I never gave up. I independently cracked 2 internships in Data Mining, MIS Executive and 1 J Operations Assistant roles. I couldn't continue due to toxic work environments. Despite my efforts, my resume isn't getting shortlisted for Data Analyst roles. I'm eager to learn, work hard, and prove myself. If anyone can refer me for a Data Analyst or related role I'd be truly grateful.

Help - how do I fight claims by scammer healthcare providers?

I recently went to 2 different providers.

Dentist, made appt via Zocdoc to see a dentist that was in network. Another dentist at the location saw me instead who was out of network. I have text evidence they said it would be covered before visit. Claim was denied - asking for ~500 dollars.

Saw a specialist who’s office staff verified my insurance would fully cover the claim. Get the claim, says they are out of network. Owe 600.

Have an offer from a dream company but I'll need to go in at a lower level. Knowing that I can bring a lot of value to the company (read- I know my worth), I negotiated with the hiring manager to have a level/comp review 6 months after employment. If I hit all my goals (ie, prove myself) within that time frame, I'll get a promotion and salary adjustment.
QUESTIONS:
Should this be in writing in the offer letter?
How detailed should it be? I assume I won't know what goals to set till I start...
