I would say that it's a good call following-up with them at this point, but I wouldn't be too worried, either. It can take some time to get everything finalized before a new hire starts work.
Did you already receive a written offer? I'd imagine so if you're filling out onboarding paperwork, but with all the job scams out there, I'd be super cautious. I do know these things can take time, but a follow up isn't such a bad idea.
A polite follow-up asking if they need anything else from you to finalize your start date is not unreasonable. If they are waiting on something, like a background check, it gives them the opportunity to let you know.
Well, not necessarily a red flag. At almost 2 weeks, there should be some kind of communication on their end, but background checks, even if fine, can take a while. Worth following up on to see what's going on.
That's bordering on "too long "in my opinion. I would definitely follow up with them but it is kind of a red flag. Things happen and people get busy, but that's a lot of time to just leave somebody wondering what's going on.
If you onboarded then they hired you. They might just be delaying the start date, but I’ve never heard of legitimate companies ghosting an employee that they hired.

Good idea to follow up. A week and a half is long enough to check in without seeming pushy. Not necessarily a red flag, but how they handle your message might tell you more.
On all my Jobs the start date was always written in the offer letter.
Sometimes corporate approval is just backed up. Call the HR person for your schedule.
