That's an interesting question. I'd say it's understood that not everything is going to work. However, there still has to be some consistency over time. Some misfires now and then won't be a huge problem, but on average there should be good performance. If something really bombs, we might try to figure out why and fix it, but mostly it's just left behind.

I’ve always been told at my company that we have a short term memory. So while we promote ‘fail fast’ we also don’t give people much runway to build a track record over many experiments
I would say we have a semi safe to fail environment. We’re encouraged to take risks, but if too many risk backfire than we need to be careful. If you take the occasional risk that occasionally backfires we are fine.
