That can happen, and from what I've seen and heard, it's happening more frequently lately. A lot of organizations are hesitant about hiring, yet they're still going through the motions. People can feel crushed that they aren't hired, yet it's got nothing to do with them, it could be the result of an unannounced hiring freeze.

What bothers me is that its not always discussed with the candidate
Something to keep in mind. Does anybody get decent feedback that better outlines the why? It could have been me, somebody better, or the job doesn’t exist anymore, all very possible.
I’d agree that’s the most common thought, but it could be anything. Good reminder to not take it personally, and just move on to the next opportunity.
