If you formally reduce your hours, note that your pay will also be lower when on your leave. That’s the main reason I opted against this.
Is there a benefit to it being formal vs informal? During my high-risk pregnancy, I met with two partners and the practice group admin and explained I couldn't keep up with the work and my health, and we made a plan to reduce the number of clients amd partners I worked with, so that there were fewer competing deadlines. It resulted in lower hours and a much simpler day-to-day of managing appointments, exhaustion and work. Nothing was formally documented through HR, but we were all in agreement on a plan.
