I try to take a call every 1-2 months to see what else is out there as it is important to me that I am “choosing to stay” vs. inertia. I am very clear with myself going in to these “what would need to be true” in order for me to entertain further. I think it’s good to build relationships with head hunters as you never know when that time will come where you are more open to something.

I take every call to hear what they have to say. Sometimes it is a fit and other times it isn’t but can’t hurt to meet the recruiter and hear them out.

Wait, what do you mean by a hard time saying no?
Does this mean you're in an offer conversation, you don't want to accept the offer, but when they make the offer to you, you can't get the words "No, thank you" out of your mouth? Or do you mean you're interested in the shiny object or the higher pay of the new role?
In any case, it sounds like the issue goes a bit deeper than "How do you handle taking recruiter outreach?"
I’m more so asking how often folks take recruiting calls. I was just giving context that every 2-3 years I’ve usually found the opportunities to be good ones and have taken them through to accepting the offer.
