I would let my employee leave and come back I would help cover them or ask them to stay longer to help pick up what was left. Honestly if they are nice they will make it work and if they try to give you a hard time they are probably super lazy

I never go to the doctor, but if I did I think I could just let people know and go. People seem to leave for appointments now and then. As long as it's a legitimate reason no one gets too concerned about it.

Oh that’s nice that’s your job seems flexible
Ask your HR department my company allows flexible scheduling for such situations. And we are allowed to do a total of 2 hrs of make up time without having to take PTO. But if you have a sucky manager they could deny the request stating “business need” but try to ask for the request a few days in advance if possible so that it could be no excuses on their end
