Did the Staff get trained at all? Or did she get thrown to the wolves and doesn’t know what to do
yea i totally get that. you can remind her that she can always ask you questions (again not technically something you have to do, but to try and help her) but at a certain point she just needs to ask questions. it sounds like you’ve already been helping her and i commend you for that. larger accounting firms should honestly just have dedicated trainers for new hires it would make things easier imo

Trust your gut and just stop working with them unless you have no choice. A few months is enough time to know for sure that they don't like working for YOU, not to say they're bad at their job just yet. They may do better jobs with others, but otherwise micromanage and half prepare because you will be blamed for the budgets.
100% long enough to know. There's entirely too many resources out there for that to be going on. Most of us got zero training, figure it out, you can't train for every scenario. Either she has critical thinking and problem solving skills or she doesn't. Give feedback, show her how to correct her mistakes, if she keeps making the same mistakes, document it, when she keeps making the same mistakes, get rid of her.

Very rarely - it’s more often a case of lack of training / guidance from whoever is delegating to them which can be addressed, but needs to be addressed quickly or it will spiral.
You need to introduce her to someone really friendly and let her have some one-on-ones with them so she can get comfortable asking her questions. Sometimes it takes a little more for introverts to come out of their shell.
She also needs direct feedback that her lack of questions are a problem, and that she needs to follow through on her work and carefully read all the steps before it becomes a bigger problem. Encouraging her to ask questions is not enough; she needs to be told directly that it's expected for her to ask and know exactly what is expected of her beyond that.
If at 6 months it's still a problem, then yes, you have a real issue.
As someone else pointed out though, it could be she is shy or unsure who to go to, or is so nervous to make the wrong decision she makes none and doesn't ask anything. She also may struggle with your communication style and training style. Maybe she would work better under somebody else? Has she been exposed enough to how her peers are working and what they are doing? Sometimes a little friendly competition and seeing what's expected of the role can help.
Check with one or two other people to calibrate and if they agree talk to HR and manage her out.
In my opinion, this scenario can depend on many factors such as lack of training or understanding. Could be that she may learn differently. Some people may learn by visualizing, talking, having a written guide, or video tutorials, etc… Have you had a conversation about this or tried following up with her on the completion of the projects?
