Focus on actionable items. I have been in HR for a long time, and what I find useful in this scenarios is giving your manager/leader something to work with. So its not "I got another offer, I am looking to leave". Its more around you being offered something you can't ignore. Pick one factor ( better comp or growth opportunity) . Something they can focus on and it does not sound like a complex scenario that is easier to walk away from, than resolve. "I was presented with an opportunity I can not ignore. With that said I really enjoy my current role and was wondering if it is something we can work towards. " It clear, but not aggressive, see what they respond with.

I've only done it when I've been asked directly. Maybe that's a mistake, I just always worry I'll come across as playing an angle and don't want that held against me, especially if it's a job I really want.

If we are talking about you getting an offer while beomg employed, do not ignore all of the reasons that you were looking elsewhere in the first place. Genuinely ask yourself, are all of these issues resolvavle in a satisfactory manner over the longterm? That answer will guide you on how much rope you want to offer your current employer in this discussion.
If we are talking about two new opportunities that you are comparing, I personally wouldn't bring it up to the preferred option, and I might try and use the leverage to see what the lesser opportunity changes to. It's an employer's market big time right now, and this kind of negotiation could blow up an offer. There are a ton of well-qualified people looking for work right now.
