Move to Costa Rica
Can you tell us what health care is like in Costa Rica?
It sounds like you need a break. It is very easy in this industry to get sucked into over working. Don't do it. Take care of yourself first and then you can better do your job.
if you feel like you are getting burned out, you probably are and it is time to take some time to Netflix and chill. No shame or blame in that.
Take a two week vacation
Sometimes it is ok to do less, refer, and delegate. Sometimes we are slaves to our own desires. Are there some expenses that you can cut down on in your life? Do you have to work so hard? Could you work less? Could you switch to contract work? I have done contract work for 2 years. I work as little or as much as I need to.
Some people seem tied down to their jobs because of health insurance and other benefits. Explore private health insurance companies and see what plans they have to offer. (Note: Healthcare insurance is not as costly as you might think.)

Are you taking care of the basics in terms of sleep, hydration, food, physical activity? It's hard with a hectic schedule, but sometimes stuff like not eating or drinking all day contributes to fatigue and is fairly fixable by doing things like pre-packing lunches on the weekend, bringing a bit bottle of water and drinking from it at scheduled intervals, stuff like that.
"They" have laid this in our lap to fix, too. Piled on, piled on admin, redundant metrics, more patients.
And then we have to fix our burnout.
