Look up ManagerMethod on Instagram she did a few reels about this this week that were well done. In short…this conversation should be had by you alone toward the end of the day and approached with empathy. It’s already an embarrassing conversation so come from a place of I want to make you aware. Do not cite complaints from others. Use “I’ve noticed” statement.

Agree with the above. Had this conversation with an employee once. The smell was due to a medication they were taking for a serious illness. Empathy, empathy, empathy. You never know what’s causing it.
It's a really hard conversation and there could be mental illness issues driving this. It has to be approached from a perspective of "I am concerned about you" and even then it may not be effective without counseling.

HR should be able to give you some prompts on what to say/not say. For example, I don’t think you can even leave an inclination that you suspect a mental health disorder. If HR didn’t give you any pointers, I would definitely find that information out. (Avoiding lawsuits)
Is this person from a different culture? When I worked in higher education, we had English second language students that had to go through American bathroom protocols.

Is this a new thing for this employee? Is it possible that there is a mental health issue, especially if the performance decline is hand in hand with this?
What does she smell like? I’m curious because if it’s BO smell then I think it’s a pretty simple convo. I would just say hey I know this is a awkward convo but I think maybe you need to try a new deodorant. But if it’s another kind of smell…well I think I would go back to HR and tell them you aren’t qualified to handle that convo.
