Could it be allergy? My dog has something similar when she doesn't take apoquel for a while
Same here, last year my lab was throwing up every night, and we couldn’t figure out what was wrong. It turned out she had an ear infection caused by allergies. I hadn’t thought of that connection before. We started her on Apoquel and also cleaning her ears regularly and that has helped.
My dog had that and the vet said it was a hot spot .... something like that.. loll . They gave me Tresaderm and it went away .

More or less, but definitely had seasonable stuff and we'd be more cautious on cleaning ears.
If you can swing the cost, go to a dermatologist.

Do they have droopy ears? My dog would get something similar from moisture being more trapped. Had a weekly wash for his ear. It was something you could get off Amazon. Otic something. It helped a lot.

Looks like yeast infection. Should head to the vet and get some meds.
Sounds like an inexperienced vet who lacks knowledge. Get another vet who knows what they’re doing.
