It's something I'm not worried about, I'm very straightforward actually, and so far I haven't had any trouble with it.
It's kind of annoying really, but I try to look for something in common with which I can change the conversation quickly and courteously.
If the person continues to ask questions about their pet, you can say something like, "I'm sorry, but I'm not able to answer specific questions about other people's pets. I would recommend contacting their veterinarian."
I think you can tell them that you are not on duty, therefore not capable of performing a proper examination. If they want to ask questions, have them schedule an appointment!
"I can't offer any advice without a formal assessment. If you would like to book one, here is the number for my clinic." For family I make it sound a little more casual of course, but it's still clear to them that the conversation is going no where.
