I’m always an advocate for time off when you need it - sick or just vacation - but assuming you’re feeling “well enough,” in this situation I’d take the call, at the beginning say something about being sick, and make this one where you’re really interviewing them. Get explicit about things like sick time, PTO, etc.
Recruiter could have been having a bad day, etc. and that interaction may not actually represent the company. If you get the same feeling after the screen remove yourself from the process.
