Are you asking as it relates to being a Registered rep of a BD or as an independent RIA? Answer differs depending If you’re affiliated with a Bd, you’ll need to get registered through the BD in each state you have a client. Likely will not need to register as a foreign LLC since you’re technically operating under the BD (even though you might have a DBA LLC. But the foreign LLC thing is state specific so you’ll want to check with the secretary of states office

Healthcare corporations… please note you have more then 1 shift of workers. You have 3 most times, each shift has to be treated equally.
