Can someone please explain why this exists in my paylslip, is it good or bad from tax perspective and shall I ask my HR to decrease it???
Please help asap.

Yes, very normal. Pregnancy isn’t an illness but it’s certainly a condition that is hard af for 9 months. Put the pregnancy first, work second. If your firm is worth a shit you’ll be back at it and folks won’t blink.
Based on my experience and others that I’ve talked to, yes this is totally in the range of normal. A lot of pregnant people experience debilitating exhaustion and nausea in the first tri that is incompatible with normal levels of work, much less big law. The way people in this bucket seem to deal with it is by building up good will beforehand (which it seems you have with your hours), using all available options for nausea meds (unisom, b6, reglan, zofran, etc), just accepting that their hours/reviews/bonus may take a hit, and taking fmla as a last resort. Sorry you’re still in the trenches though—first trimester was absolutely miserable for me as well. Wishing you comparatively easy 2nd and 3rd trimesters!
I didn’t slow down at all and was on pace to bill 2,300 hours when I was pregnant. I was so sick all of the time. I ended up having my baby 3 months early and I really think the stress contributed to it. Take the time and don’t push yourself.
Yes and extra hard when you aren’t ready to tell work about the pregnancy. I’d just try to get meds etc and hang in there. If you are ok saying something to a trusted partner even though it’s early, maybe that’s an option just to get extra support
