ACDs are basically just senior creatives. They mentor juniors and interns and direct them at times, but they’re also usually a creative team making the work.
I specify copy so people know my background and don’t contact me expecting some high level of design experience.
It means you’re a senior copywriter who finally got a slight pay rise and now has to do all their own mood boards
Both are possible. Some agencies only have one and some have one each for both teams. But in general, if you want to be an ACD, you should expand your horizons instead of fitting into just one box. It'll do you good to have your ears and eyes open for all that makes good advertising.
Yes I was once an ACD CW. I got promoted to VP ACD CW - that’s a lot of alphabet. There was no CD spot open, but I won a lot of awards that year. It depends on the agency structure.
VP ACD of copy (CW) or art is super common! And yes, usually still in pairs as CD but you need to know about both. Above VP CD is, more often, one or the other.
An ACD Copywriter will be able to give direction to an art director (maybe not at the granular level of an ACD-Art). They just wouldn’t be able to implement it on their own if they had to.

In some small shops, you'll have an ACD and/or CD that can do both.
But in a lot of shops, there are ACD and CD teams who still have their focus -- art direction or copywriting.
My agency has just one ACD and one CD. Not all these intermediate roles
