The only thing I’ll say is always, always leave on a high note. You never know who you’ll encounter again at another agency or who you’ll need as a reference who knows whom when you need an intro. Never say never.
D1 Nothing official but for sure the details of exit interviews go to the Managers Manager, sometimes gets discussed at that level if anything of note, and could harm the individual’s reputation with that small group of potentially influential people. IE decision makers. (Especially if you assume that people’s seniority relative to age cohorts doesn’t vary that much over time, so in the musical chairs of adland the same people may well be on the end of a hire decision of the same person in a different agency at a different point in time.)
I say this, as in multiple organisations I’ve been in the forums where this information comes up. Usually a leadership team meeting where the leaver is flagged and the group wants to know why and any associated context. Anything beyond a dull response will get discussed.
Yeah that’s a no. Fill out the form and if they want to chat, the onus is on them.

Maybe they mean schedule yourself 30 minutes to complete the form? As in, take time out of your last day and let us know when youre ready?
Not taking their side, Im sure theyre terrible, but that would be a nice spin on it

Schedule 30 mins on her lunch hour, the meeting will never happen.
