No, you have two options from here:
1) Join the tenderloin population
2) Run away to NYC and live among the mutant street rats
In all honestly, I doubt you’re in a bad spot unless you committed a crime.
Probably not, but it depends what you did and how broadly it was known. Simply being incompetent will have much less impact than e.g. if you were doing something illegal.
If you apply to a new job, and there's people you previously worked with there, it's fairly likely that they will be asked about you during the process. If you left a very negative impression, that probably won't go well. But there are surely lots of companies where you won't have any former coworkers.
Another thing to consider is that many places ask for references and do call them. If you can't provide any positive references for either of your last two roles, that's going to raise some eyebrows. But you at least only have to worry about this once (assuming you don't "ruin your reputation" at the next company, at least).

Everyone has said something dumb in meetings. The stuff you should be afraid of isn't where you think you said something dumb, it is where you were too uninformed/inexperienced to realize you said something dumb. The good news is that people don't care or remember much.
You should be fine unless you work for a super niche field and you're staying in that field for your job. Then maybe switch fields to be safe?
