I think a lot of it is perpetuated. I make it a point to tell junior employees to go home if there truly isn't anything happening.
My VP once told me,
"The bank is run by people with short term goals. They want to get in, make changes that will get immediate results within a couple of years, then get out with a nice big mountain of cash and a good rep. They aren't interested in investing in an efficiency change that might take a while to implement, even if it means potentially huge long term gains, because the losses or stalls in profit while those changes come in make them look bad. Doing slightly better this year will trump doing average this year and 2x better in 2 years time or 10x better in 3 years time."
Made a hell of a lot of the bullshit make sense.
Because it's a culture that values the appearance of work. Honestly, a lot of the work does not require that much creativity and just a reasonably smart person to go through the motions. As a result, there's no way to separate person A from person B other than FaceTime. So all the overachievers in the field pile into staying late to appear to be dedicated and committed, or adding dumb comments on a draft that didn't need it to be seen contributing. This creates insane inefficiency, but no one wants to reveal the true inner workings and pull the cover back.
