Discrimination. Sexism, racism and aging out.
Grown adults spending their time and energy making pretend work to win awards given out by juries who won’t talk about how it’s pretend because it will expose their own pretend work and no one in the industry is allowed to talk about it publicly because for some insane reason it’s considered graceless.
People that don’t communicate well and often.

Ageism, lots of terrible managers/leaders, favoritism, lots of hacks, rapidly diminishing job security with everything moving to projects, influx of non strategic account folks who are glorified yes people, myopic focus on having specific sector experience, ironically hardly any priority on building agency brands.
Over-complicating work so it “stands out” just for it to be more noise that people desperately try to skip.
Believing the main strategic value is in the framework or model, and a scarcity mindset in sharing knowledge, examples, or tools.
“Shareholder value” obsession making clients and agencies alike make desperately stupid, short-term decisions.
When HR ghosts you mid way through the interview process. It’s not hard to give a human being an update. The ultimate irony is that we would be fired in a heartbeat if we ghosted clients.
The disrespect between departments.
