Not a documentation tool, but I think Salto is great for analyzing metadata/diff.
No - you need a tool or an application. Not excel, not word. Those will never get used and will be plagued by tons of user error. Either a native app, 3rd-party tool, or something homegrown. Talk to other depts. about how they handle data dictionary type of cataloging.
There are tons of paid apps on the app exchange. You can attempt to build one yourself. I'd ask what others are using. You need to define the purpose and the goal and how it will be maintained/define an update plan. If those things are not outlined it will go downhill quickly. The reason I say for it to be native is because it makes adoption that much easier, and makes it easier for the developers/architects.
If you already have Jira, I would recommend Confluence. It’s an Atlassian tool that links to your Jira where you can organize your documentation.
At this time I use Sonar. I can give a user
Access as it is read-only. That info will be read and displayed assuming it was added during setup. Sonar also offers a data dictionary add-on to make updates to descriptions, ... I’m both surprised and not surprised that SF doesn't offer data dictionary functionality.
This sounds like a data dictionary and it is usually in Excel. Someone has to own keeping it up to date though.
No excel is terrible for this even though it is what usually gets used.
