This may be an ignorant answer but maybe supply and demand? Demand is higher than supply here in the US, demand is lower than supply in India. Just a guess though
This is part of the story. A higher degree in the US also costs an absolute ton of money. Education can be expensive the world over but in the US it's eye watering.
A US undergrad degree can cost roughly 17x the average Indian medical writer's salary.
If you look at the ratio for the average medical writer salary vs the average graduate salary on a nation by nation basis, they aren't wildly different in India vs the USA. The ratio for the UK is actually poorer than either.
Medical writers don't get paid "really well" in the US. Entry level salaries barely cover expenses in HCOL cities.
It's not quite so much about subsidising the rest of the world as it is high rate of return and (often) setting new standards of care. If you focus on launching a drug elsewhere and as a result a competitor gets FDA approval first, you've likely lost the race.
