A dog comes in with some teeth that are rotting in the back. I tell the owners that they need to be removed. The owners say they don’t want to remove them because of the cost. They tell me the dog is eating just fine, acts fine, and shows no signs that he is in distress. The dog came in again for his annual visit and again I mentioned the teeth needing to be pulled but they still refuse. Should I continue to push or just let it go? The dog otherwise is in excellent health, they bring him in and gets all his other shots/immunizations/flea meds/etc.

Don’t push it , just remind them that’s it recommended. Especially if this is the 3rd time . Then note it for records .

I would recommend you to let it go since they still have the final say regarding the matter. Just make sure that the problem is indicated in the dog's records. Your heart is in a good place but if they do not want the operation we can never push it on them.
