What do you mean by burned bridges? Did you drop a ball of any significant consequence, or did you just mildly piss people off? If the latter, there’s probably a path to redemption, but you first need to take care of the cause of your burnout and take care of yourself.

Yeah this doesn’t sound that bad? It sounds like you may just need a reset. Maybe take some leave and start therapy and/or get a coach. Also don’t ignore the possibility that your nervous system is telling you this job isn’t for you long term, and that’s OK. Be prepared to lateral or go in house if things don’t improve.

Am I nuts or is this really not a big deal? Like if you were always responsive and hit deadlines before this, needing follow up reminders during a three week period is really not bad at all lol. But I would take care of that burnout ASAP
Agreed. None of this seems to rise to having burnt bridges. OP—Sometimes life happens and you aren’t performing at 100%! Address your burnout but don’t beat yourself up too much for a rough couple weeks

Take short term disability leave. I would work with a therapist/psychiatrist and also be screened for undiagnosed physical health concerns by a doctor.
This process may make you reconsider staying at this job, so I would explore a career change, too.
I guarantee you’re imagining 90% of the reputational damage. Just lock back in and you’re fine.
What year are you? The more junior the easier it’ll be to recover
