Look at types of projects - both clinical areas and functional. For instance, historically Putnam was very heavy on bipharma and vaccine, but not much DH/medtech/Dx. LEK does a lot of CDD, but has a strong medtech practice. ZS historically did a lot of salesforce re-org. Health Advances has one of the best Dx practices in the industry.
Look at work life balance... read the vault guide, fishbowl, glassdoor reviews. Firms like CV and LEK have notoriously bad reputations here. Personal experience says firms like Health Advances, Kx Advisors, maybe blue matter, and SciVida have better work life balance.
In terms of comp... the firms that have poor work life balance and better exits have slightly higher comp. The smaller boutiques are a bit lower on comp... but Look at vault for business outlook... since that might predict your bonus, not all the small firms growing at equal rates.
At smaller firms, it's about fit. How do you feel when you talk to the team, the leaders. Every firm has a personality, and without understanding what it is, it's hard to know if you'll feel good there.
You can be successful anywhere, you can build a successful career in all these places. If all you're searching for is an exit before you even get there... reflect on why.
Honestly, I came into consulting thinking I would do it for 2 years, but I fell in love with the work. I have been blessed with fantastic mentors along the road, talented and kind coworkers, I'm glad I picked based on where I fit.

Syneos, Eversana, and Blue Matter aren’t bad if you want a smaller environment, ZS and LEK are both pretty large firms and culture is a lot more intense
As most have mentioned, LEK and ZS are probably consensus top 2. LEK pay ramps up faster, but they will make you work hard for it. ZS also pays well and is more sustainable, but still going to be consulting hours though
Can you say more about the specialties? A lot of the statements in this thread are very broad
Clearview, ZS, LEK will be the most recognized. Comp is pretty standard across the board ranging from 80-105k ish. 50-70 hour weeks as a junior is also expected. LEK would have the worst culture out of these three

Best depends on what you prefer. A firm that does pure commercial or market access strategy when your preference is portfolio, clinical trial, and asset level strategy isn’t going to be the best for you regardless of if its prestige on a consulting firm tier list is higher. And when you have to compete with someone from a lower tier firm in an interview for an exit in that specialty that they did a million projects in, you’re not going to look better than them.

Don’t worry about pay. 10-20k per year at the entry level won’t change your life in the long run. Find a firm that fit best your interests i.e. exits, growth, prestige, etc.
