Help - how do I fight claims by scammer healthcare providers?

I recently went to 2 different providers.

Dentist, made appt via Zocdoc to see a dentist that was in network. Another dentist at the location saw me instead who was out of network. I have text evidence they said it would be covered before visit. Claim was denied - asking for ~500 dollars.

Saw a specialist who’s office staff verified my insurance would fully cover the claim. Get the claim, says they are out of network. Owe 600.

After rejecting big companies like Infosys,Wipro, etc. I've joined EPAM Anywhere due to wfh permanently and it's perks. But after spending 1 and half month HR called me to search the opportunity outside of EPAM, means they're laying off people. It's unfair behaviour of the organisation. I am thinking of resigning from my company due to toxic work culture and no work life balance.
I need to know the notice period.
I am currently in probation period and there is still 1 month left for my probation to end.
In my offer letter it is mentioned that any probationary employee should give 1 month written notice before leaving. So I am confused if it is 90 days or 1 month.

Hi Fishes, I've 2.25 Years of experience in SAP Basis, but I've lost interest in the same due to not receiving proper training, inability to practice on customer's system and shifts. I'm looking forward to go for MBA/changing technology, which one would be a better option?
