It depends on a number of factors.. The pay band accounts for things such as years of experience, performance ratings and location. If you work for a large company with offices across the country, then location will likely play the most significant role into where you land in a pay band…
For example: Let’s assume you work for a national firm that has offices all across the country and the Senior Underwriter pay band is $100,000 - $200,000. The pay band is that wide because the company is attempting to pay everyone holding that role in an equitable fashion. If you’re a Senior Underwriter in Birmingham, Alabama (low cost of living), then you will find yourself on the lower end of the pay band because your cost of living is significantly lower than the Senior Underwriter who is living in Manhattan (high cost of living). Even if you are the most tenured and highest performing senior underwriter, i’d guess that it is still unlikely for you to ‘max out’ the pay band because there is such a discrepancy in the cost of living adjustment.

Yes. At least at my company, each position has its own pay band. For example, the pay band for associates will be different than the pay band for senior associates.
If I recall correctly, we even have different pay bands across for each service lines as well: the pay band for consulting associates will be different than audit associates. However, I do not think it gets disaggregated more than that, like there isn’t pay band specifically for technical accounting consulting associates vs risk management consulting associates but I could be wrong as our IT consulting folks likely get paid the most
