You may want to look into a cruise. Retirees LOVE them, so many have excursions that are geared toward older individuals. And if your parents have mobility issues there's plenty of entertainment on-board so they won't even have to get off the ship if they dont want to. They also have medical staff on-board who can assist with any minor medical issues that could pop up.
Cruise is a great idea. Cruise staff can also help plan and accommodate your parents disability whether that be on board the ship or through various on land excursions.
A lot of European cities are older and may not have the same ADA accommodations that are in the US. Also, are you planning on going with them? Could be hard for them to get around without a guide or chaperone.
Also, is this what they want? Only asking because my grandparents several years ago asked us to take them on a trip. They turned into slugs and didn’t want to leave the hotel room. It was the oddest thing. Even for dinner they didn’t want to come out. I think they were overwhelmed but it all and ended up ruining the trip for everyone.

Most older cities will be hard to navigate as a disabled person. As others, I would HUGHLY suggest an European cruise. My grandparents took multiple Viking cruises (Christmas, Paris to alps, grand European, and of course Viking shores) and they love them. Norwegian is also good.
I wouldn't suggest Lisbon or Porto (in Portugal) or these are VERY hilly cities, so much hat there are utube bloggers showing you exactly what it is like. Depending on the (disabilities) Croatia is absolutely beautiful Zagreb and Split are both flat, main squares are easily accessible. I also enjoyed Vienna another beautiful spot
Definitely a Mediterranean cruise or Viking cruise.
I am close to their age with health issues and I would love that.
Alternatively, consider Florence Italy. It has lots to do without leaving the city and works well for disabled people. Just be sure to book a hotel on level ground as there are a few hills there.
