Normally it would go to your practice group leader, but if you're in a smaller firm, I would send to whomever you consider your manager and to HR. About to lateral myself and I plan to tell some of the partners I work with closely first as a courtesy so they hear it directly from me. As for the notice period, if you have no formal requirement the only consideration would be how long you'll need to transition projects. If you're not wanting to burn bridges, make sure you leave enough time to at least smoothly close out or get things onto other people's plates.

It's good form to notify whomever your manager would be, presumably one of the partners, and cc someone in HR. You might also have a conversation with the partner you most often work with. As for the notice period, you don't want to burn bridges or create bad feelings, even if you consider the firm toxic. Always leave on the best terms you can, don't lower yourself to the toxic level, so make sure things can be transitioned.
I agree with Attorney 1, I would send it to your manager or whoever is above you and HR. You don't owe anyone two weeks, that is a courtesy. If you can't give them two weeks, so be it. At the end of the day you have to do what is best for you.
