Great choice, did similar before my current job. I'd recommend a rainforest night tour, sloth sanctuary, hanging canopy bridges, volcano hike, hot springs spa, coffee/chocolate farm tour, ATV excursion, and surf lessons if you're feeling adventurous. Amazing pura vida times

You've got some good recommendations already, but I want to cast my vote for Monteverde Cloud Forest. It's a pain to get there, though, so I could understand saving it for your next visit.

From SJO I’d recommend going to Jaco/Playa Hermosa and Manuel Antonio. Take a guided tour in Manuel Antonio because the guides know where the animals are and have fancy equipment to take cool pics. If you’re adventurous there are zip lining experiences in that area and do a kayak tour with Kayak Jaco. I didn’t do it on my trip, but I’ve heard great things about Nauyaca Waterfall Nature Park as well
Second the extra insurance! The roads are not great at all and we would’ve wound up paying a ton if not for full coverage. Also would highly recommend baldi hot springs

I’m going later this year but splitting time between La Fortuna / Arsenal and Guanacaste (but flying out of LIR)
Assuming your are flying into San Jose. We had friends who built a house to retire in, and weren't interested in seeing the resorts, etc. up north so we always headed south for a majority of the time we spent in CR. If we were to go back right now, we'd head somewhere like Dominical, Uvite, Playa Ventanas, possibly even more south. Domincal offers a great beach and easy access to Manuel Antonio and San Isidro De El General inland. Farther south you can go to Palmar Norte or Sierpe, which gets you clsoer to even more wilds that look like scenes from "Jurassic Park". Plenty to do as long as you don't mind outdoor stuff, stomping around jungles and hanging out on beaches.

It has been 15 years but I loved Costa Rica. My favorite areas were Manuel Antonio and the Guanacaste region. Arenal had some cool adventure things around it and the hot springs are novel but would go back to the first two places I mentioned with more enthusiasm.

A day trip from SJO to Irazu volcano was breathtaking. walk in the crater was Amazing.
