Nah, it’s right. It’s referring to the 1, not the hundred. I bumped on it slightly on first read, as well.
The sentence and meaning is '1 kid is medically complex (out of 100).' Not '100 kids are medically complex.' 'Is' is correct.
This is grammatically incorrect, right? “1 in 100” describes the kids (plural), so it should read “1 in 100 kids are medically complex.”

There seems to be some disagreement online about the rule. But I agree with you. “1 in 100 kids” is not referring to a single child. In Canada, 1 in 100 would equate to thousands of children. So in every sense, it’s plural.
