Cleared Accenture Skill interview by answering almost all the questions and received congratulatory mail for HR round and submitting all documents. Then after a week received a call from HR saying that due to some technical issue, the skills round will happen again and an online assessment was set up. In the assessment, questions which were not directly related to the profile were asked and then a rejection mail was received. Does this happen a lot?

Yes. I have been applying for jobs all year most with no reply no responses no ability to even ask for suggestions. All that said I had about four recruiters. Tell me my résumé needed to include quantitive information. Unfortunately since this is never something people think about unless they’re in sales or have highly influential walls. So I had to think back over the course of a decade or more at the same company with two different roles and guess at percentages based on my role and individual contributions. Is it 100% factual yes and no to the best of my ability. Hope that helps.

Definitely. That's great practice for performance reviews. If you're asking for a raise, you need to be able to cite your accomplishments as justification to have any hope of it getting approved.
