Definitely put in the work and your role.
For the third script, I'd personally lean toward including it at the end as long as you clearly state the facts.
The reason I'd include it is because it sounds like you came up with a solid platform that can continue running in theoretically endless executions — but it's your platform idea, and that's the hardest part of our job. Bonus points that it was the pitch winner too.

I assume you have a decent relationship with the CD. If you feel conflicted about it, just ask. If they say no, just ignore.

Were you the writer or the AD? It was your idea?
I was AD in the team. The platform/concept was my partner’s and I, two of the produced script were ours with basically no changes (accept of course the stuff that happens on set and actor improv etc) and a third script came up as they were working with the director I believe, and not from a new round of work.
Put in your work first or the as the main feature on your book. I would just link the third as a link.
