All the time, but my office is pretty laid back. You'll get more useful info just by looking at what other parents at your company do. If they regularly take off early for appointments, you should have no issues doing the same.
I'm able to take time off for appointments, and I assume that's because it doesn't come up very often. I don't have kids, but if I did, having to pick them up from school might be a problem. When I was a kid I took the bus home and let myself in, as my parents would be working. I'd hope my own kids could handle that the same way.

Lots of districts don’t have enough buses. That’s my main worry for when i have kids.

At my job it’s pretty flexible - taking time for appointments or school pick-ups isn’t a big deal as long as the work gets done. It really depends on the manager, but for us it’s kind of the norm.
