You can not hire an H-1B unless you can document no american with the skills for the job was available. That’s the law you are in a tough spot even with the market again on fire especially in professional services.

That was a great economic report on Friday, but H-1B will lag because the law is the law

A patient of mine essentially asked me to fraudulently bill his insurance the other day. My response was records can be audited at anytime which could result in my license being put at risk. He asked me how many times has that happened? I was like that doesn’t matter. I spent way too much time, money and effort to get to where I am.
