Catching those errors used to be my job at ESPN. A lot go through but we were good at fixing many. I still see a lot, especially on TV. The biggest one I can think of recently was in the Spider Man 2 video game.
One of the main characters is of Puerto Rican descent and instead of using the PR flag throughout the game, they used the Cuban flag. People were hot but they fixed it within the weekend.
