I interviewed with Firm A months ago and accepted a position to start this upcoming fall. Group leader called me to offer the position back in November 2020. Besides interviewing with partners at the firms, and diligent follow ups notifying them of my interest to get started, no communication has transpired since. It turns out that I accepted a better offer w/ Firm B for personal reasons and won’t be going to Firm A. What is the best way to notify Firm A? Call/email group leader? Call/email HR?

I’m in a similar boat (from LatAm, jumping into the USA job market) although I’ve been getting more interviews / offers lately. However, my main message is: please do not despair. The job market is challenging and the average time it currently takes to find a job is 6 months. I only started to get some answers after 3 months of perfecting my job search strategy. Happy to share a few more pointers and also hear about your timelines on GC application. Feel free to DM me.
Are you making it clear on your resume/application that you're a legal US resident with work authorization? People scan resumes quickly and could be assuming that you'll need sponsorship and/or aren't in the US if all your previous work history is overseas.
my friend when moved to States spent 1 year looking for a job, but tgen he got 2 offers simultaneously

It's probably because you're not a citizen, unfortunately because of the volume and volatility of immigration approvals in the US even people with temporary work permits are having a hard time right now! Don't take it personally and keep pursuing your career goals. If you're having better luck in your home country, that might be the place to be for now
