It can be a real challenge!! IMO, the best approach is to emphasize the importance of the job and to be empathetic. Instead of using a three strikes and you're out approach, I prefer to focus on identifying the underlying cause of the laziness, and then working with the employee to come up with a solution.
I agree with this approach. I would want to know what the underlying cause is. Is it something that policy or procedure tolerates, moral, or lack of direction. If none of the above then clear direction and understanding of job responsibilities and why the role is important to the success of the organization would be good for the employee to understand.
Several ways to approach this, I lean toward understanding and deeper thinking before strikes.

When I was 18 I was lazy working in a FedEx warehouse. The manager called said he needed to speak to me in the office. We did, I sat down, he sat down. He shuffled some papers on his desk, looked at me, and said, “Do you want to continue working here?”. I was dumbfounded, never really confronted like this before. After this meeting where he explained everything I did wrong, and what the expectation is moving forward, I wisened up and years later he joked that after that meeting I became one of the best employees he had. I think a serious worker, when confronted with this reality, will realize the error of their ways and course correct.
Yes three strikes and you are out. You rae here to work and things need to get done. Sorry, we need to run a business here. You don't want to work other people are looking for jobs as well who are willing to work.
