If it’s a small company, or privately held, often times the job requirements are dictated by the leadership/CEO, who obviously have unrealistic views of what the requirements are. Don’t blame it all on HR.

My favorite!!! We also pull the same shit in proposals or orals presentations I heard two people say they have been implementing a SaaS platform for 20 years that was initially released in 2014 and I know Accenture didn't really start implementing until 2016!!!
