I feel for you. I’ve been there before and periodically fall back into that headspace.
Have a sit down with you to identify the root cause of why this is happening and be honest with yourself. I don’t take medication so it takes some serious effort to stay organized. Are you checking the easy boxes - staying hydrated, working out, cleaning your workspace, and removing distractions? Are there things in your personal life that has your attention? If so, do you need to take time off to handle that issue? Helps to remove any mental load.
Also you mentioned you jump from one call to another so really think about how you are preparing for the day and each call.
My notes were a mess until I began to use a standard template which I would then prepopulate (agenda, background, attendees, questions I have, action item section) ahead of certain calls. Copy what you can from the invite. It requires sometime upfront but saves you on the backend. This has helped me tremendously to not confuse speakers, establish a designated action item section and capture key points.

I always send a polished version of summary + action items after a call via slack/email to get it double checked + have deadlines + contact details double checked - that way you’re on the same page
I’ve used trello to keep track of all engagements so I pop all notes on a template like google docs or Microsoft one note (click the ‘meeting’ setting https://support.microsoft.com/en-us/office/take-notes-in-a-meeting-ad35c845-f9ff-46f7-bbe2-7bcc70f4c99e) in date order/folders for separate engagements. You can add deadlines to all trello tickets etc to see what’s the priority etc
Take 2 days off or a whole day off and use that time to set up systems. Such as a new notes application you can find and like better. Things like that so you can perform better and be less overwhelmed.
I don't have any practical advice since I'm not diagnosed but I resonate so much with you. I feel like I could have written this exact post!!
OP do you take medication? That was the only thing that worked for me when I was feeling this way. I honestly felt like I was drowning before I was on medications and now I can do things like a normal human being.
I am on medication but because of the shortages, i’ve been taking it every second day or skipping a few days to make it stretch further. That could also be part of it. I was able to get a month filled just a couple of days ago so if I stay consistent again, maybe that will help.
