Whoa. I would love to be able to give you advice but I’m having a hard time understanding your post...
Best to worst: gold standard, almost always, usually, often, sometimes, rarely, never.
Each rating is “how often” you displayed: Quality/technical excellence, client leadership, team leadership, personal leadership, engagement/project metrics.
If you counselor doesn’t even know then it’s hard to make a call here. One negative review usually isn’t enough. If your other teams were happy with performance and you haven’t been dropped from multiple teams, should be fine.
Was the negative review fair? What led to you receiving that review?
I would talk to your counselor about recourse to correct that feedback.
