Red flag, look elsewhere. Pay is for skill, equity is for loyalty.
If they cared about loyalty like they are claiming, you would be getting annual Cost of Living raises. They care about getting as much labor out of you for as little $$$ as possible.

The easiest way to increase salary or title is by switching companies. The question is how loyal is the company to you? Is this your dream job and dream company?
The company views you as a number, there’s no loyalty on their side
It depends on the metrics. Do you have the data to show you pay vs. the industry’s average in your geographic location? If so, did you communicate it with the value you have brought.
If you did all of that and they still deny your request then Yes!
