Lack of consequences and enforcement mechanisms, along with competing priorities. There are a large number of laws and regulations that apply to the government that the government flat out ignore because there are no consequences for not following them and other things are deemed higher priority.

My first role in the fed sector way back in the early 2000s was as a 508 compliance tester and things haven't measurably changed since then.
