In our case designers deliver their work with a demo at the mid and end of sprint cycle. Everyone and anyone is welcome to join these. Each designer shows off their work and speaks to their thought process. People have a chance to ask questions or comment on it right then and there, in a collaborative way. Once all is done and finished the designs are handed off to Development via something like Zeplin.
I can’t tell how many do this, but definitively a good UX professional will attach detailed notes of their work (this is not the majority in my opinion). It’s very easy to spot bad work by asking questions to the designer about their choices and see if there was any thought process at all that can support your goals. You will never like someone delivering something to you that just looks pretty. There are plenty of graphic designers for that but with UX you want to take the right decision, or I would not consider that UX.

I’ve documented how things work in the form of functional specs, but generally never need to document my decision making rationale unless there are questions and I respond to them specifically in email.
When I present work I give rationale and will specifically call things out that need explaining but generally I’ve never needed to give a full, deep explanation. Most of my clients know me pretty well so they trust my expertise. For new clients I’ll ask for longer meetings to explain, but documenting it all feels like time that could better be spent getting a product out there and testing it.
I probably wouldn’t get granular with things on email anymore like I did when I was more junior. Back then I was convincing myself as much as the client. Now I just reiterate what discovery documentation taught me and how that resulted in my work while I present. I would do it for a client that wasn’t convinced.
