1. Manager has taken performance/behavior concerns to HR.
2. Manager has followed HR's guidance to the letter, usually over several months, and performance/behavior has not sufficiently improved.
3. HR agrees that termination is warranted, and conducts necessary termination procedures.
If you're asking what performance/behavior concerns warrant a conversation with HR, it's any action (or lack thereof) that inhibits job performance at the expected level. Can range from incompetence, to disrespect, to simply being irresponsible. Illegal activity or other egregious offenses may warrant immediate termination.
In any case, when firing someone becomes a considered option, it's time to go to HR - for manager or subordinate.
