You have hit the nail on the head... There is a great deal of confusion across these roles. I think some of that has to do with the size of the organization. If they don't have the demand to have distinct roles, they tend to combine them. There are also organizations that have legacy definitions they try to attach to SF roles. Here is my take:
Product/Platform Owner - The person who "owns" the overall strategy and priorities for the use of Salesforce within the enterprise. They typically report to the Exec Sponsor of the Salesforce relationship. Also could be the COE leader.
Product Manager - "Owns" one or more "capabilities" (Sales, Service, etc.) within the Platform and drives the strategy and backlog. Reports to the Product/Platform Owner. Could exist in the Business or IT.
Technical Project Manager - Owns the "project management" responsibilities for the execution of given project on the Platform. Usually is an IT role.
However, I wouldn't get hung up on titles for the roles. I would be more focused on the critical responsibilities they execute and making sure someone is accountable for those responsibilities, regardless of title.
