CEO of what/where?
Because the board of directors voted him/her out.
CEO's get fired all the time because they are not meeting or exceeding the expectations of the board members. A lot of CEO's get hired because during the interviews they make it seem like they can do the job, but once they get hired that is when their weaknesses show. It might take a couple months or years for all the weaknesses to show. You have to be very knowledgeable, intelligent, and adaptive to be a high performing CEO and everyone does not have what it takes to run a huge corporation even if they are ivy-league.

Tons of reasons and it of course it depends. If CEO hired to fix a struggling company, they may have limited time before the Board is looking for the next messiah. If company starts to struggle, Board may decide we need someone to ensure we never struggle. The average CEO tenure is 5 years. I’m of the opinion a CEO has 2 years at max to demonstrate a path to success before the board is gathering a short list of replacements.
Bad performance of company due they leadership, poor decision making, reputational harm caused by them, loads of reasons, why does any employee get fired? Same reasons apply to firing CEOs.
Or for standing up to the wrong people at the wrong time (employee that is) 🤭
