I've always thought things like that were kind of silly, I mean, how would the person at the top of the organization be responsible for what happened on the ground? But I suppose in some ways it makes sense, the culture comes from the top. So the top person is ultimately responsible.

I see your point. If the person at the top has created a culture that encourages bad behavior I.e. racism in a police department, then yes the person at the top should be fired when someone on the ground messes up.
